How to Make a Claim

To submit a claim, download and print the claim form (PDF), complete it in full, and submit it to Ingle International.
You have access to the plan's Medical Doctor Network, a network of physicians, medical clinics and pharmacies where the student is not required to pay up-front.
Students are also free to visit any healthcare provider that they choose. If the provider does not accept direct payment, you may be required to pay up-front. In this case, keep your original receipts and submit them for reimbursement along with a completed, signed claim form. You will generally not be asked to pay up-front in the event of a hospitalization or other major procedure.
In the event of a major emergency or a hospitalization, you must contact the Emergency Assistance Department:

Your policy may limit benefits in the event that you do not call Emergency Assistance.
